This is an absolutely brilliant tool, not only is our team more aware of individual residents risks they also have a heightened understanding of the strategies to minimise these risks at both an organisational and individual resident level. I would recommend this tool to aged care services both small and large.

Natasha Chadwick Founder and CEO, New Direction

Firstly the High Impact Risk tool and factsheets are fantastic and we have already made available to the broader RN population. Secondly, I wanted to personally thank you for the training workshop on. Everyone has been raving about how valuable the day was and how impressive the Anchor team was. It was both informative and insightful and gave the team a boost. It was more than I had expected, meets my goals of openness and transparency (discussing the audit results), increasing overall team capability, and also gave a very valuable reality-check that aged care is HARD right now. The team loved the day, and I think we should aim to pencil in something similar every 3 months or so.

How do you manage your High Impact Clinical Risk in aged care?

Australians are living longer.

One in six people living in Australia in 2018 were aged over 65 years. By 2058 it is estimated this figure will rise to one in five.

It’s clear that residential aged care is going to become increasingly vital in assisting and supporting Australians in their ageing journey; if it hasn’t already.

People entering residential aged care are older, frailer, have a higher number of comorbidities and have varying levels of care and service delivery expectations.

This means consumer care also needs to become varied and bespoke to better service residents, but crucially reduce high impact clinical risk.

Consumer expectations are rightly rising, which is putting an upward pressure at board and management level to ensure expectations align with actual care and services.

However, at the moment there is a gap between the two, which highlights the tension between consumer expectations safety, quality, and risk.

At Anchor Excellence we’ve just developed a program (AnchorAssure), which is leading the way in mitigating high impact clinical risk.

It gives you the key recommendations, knowledge and critically the ‘situational awareness’ to mitigate risk. It also features a pioneering ‘profiling tool’ allowing you to individually profile a consumer’s level of clinical risk.

Whilst risk cannot be eliminated, it must be mitigated. If not, these high impact clinical risks can lead to serious harm to consumers and place the residential aged care home at the increased likelihood of litigation, serious risk or sanctions.

Decision Making in Australian Aged Care – Definitions

It’s important to note that decision making, in the Australian aged care context, is multilayered.

A consumer has the right to consent to or refuse treatment of care and services if they have the capacity to make decisions. If a consumer lacks capacity to make decisions for themselves in respect of treatment, a substitute decision maker can make decisions on their behalf.

The Accreditation Standards and The Charter of Aged Care Rights support the right of all consumers, regardless of their capacity to make decisions, to be included in any decision making about their care and services planning and delivery.

There are some key concepts around decision making in residential aged care. The following document outlines Anchor Excellence’s definition of these fundamental terms.
